games with glitches that prevent you from progressing past a certain point, especially those in which the area is a required path.

made even worse with those that require you to restart the game entirely in order to get past this point.

(God of war 2 has a glitch like this, that can show up at random points.)

the way games get released nowadays full of bugs but the developers and publishers don't even care because they can update the game

except games go down in price so there's no reason to buy it on release- you just wait a year and you'll be able to get it for $10- if there's any bugs/glitches/etc left at that point the developers probably aren't going to fix them

multiple endings that have zero effort to them. I hear Mass Effect 3 is really bad with this- or rather I know it is, even though I've never played any of those games. An example of a game that I have played that does a poor job though is Deus Ex: Human Revolution where you beat the final boss and are in a room with 4 buttons you can press that give you a different ending right away. Really bad. The original Deus Ex game had multiple endings as well, and you only got to "choose" them at the final level but at least there was different gameplay choices you had to make to get them!

I am perfectly fine with a single good ending. Don't bother with multiple endings if you're not going to do a good job.

Puzzles that are extremely easy to accidentally make unwinnable, resulting in you having to restart the level.

Also, when you can choose between multiple classes/characters/abilities and the game basically forces you to play as one and not the others because only one is not useless/only one is plot relevant at the moment. Especially if you didn't spend a lot of time levelling up that character...
