In this game, you play as a living puppet in a world of living puppets. Of course, he's no ordinary puppet. His head is detachable and it can be changed whenever he finds the Headcase character. There's quite a bit of fun stuff to do, from practice areas at the beginning to basketball mini-games throughout. The bosses are all very, very interesting creations, and they vary in style as much as the stages do. The scenery is all stage-like, fitting with the puppet theme - and as such the game makes great use of that, including a mini-boss in a sky-like setting that's hung from a giant claw. That's not even scratching the surface as far as the art in this game goes. The game play style changes in many levels; a majority of it is good platforming, but there are times where you're flying (once against your will), and there's a couple of stages that are pseudo-3D. It starts off relatively easy, but it can get to be really hard in the later stages, especially from "Fly Hard" onwards. It's really fun. There's way more that can be said but I'm not all that good with phrasing things.
Have you ever played it? If not, and if you can manage to get your hands on a way to play this, you should; the Genesis version is the one I know the most about. It's a fantastic game.
